Save time at check-in by completing the new-patient packet before your visit. Forms are available through the portal, by phone, or at the front desk — whichever is easiest.

Three ways
The new-patient packet covers the basics — health history, insurance, consents, and parent contact info. About 10 to 15 minutes.
Once your appointment is scheduled, we send the new-patient packet to the portal. Sign in, fill it out on your phone, submit before the visit.
Call us and we’ll email or text the forms to you, or we’ll set them aside at the front desk so you can fill them out when you arrive.
Prefer to do it in person? Arrive 15 minutes early and our front-office team will help you through everything in English or Spanish.
