Three ways
Get the forms in the way that works for you
The new-patient packet covers the basics — health history, insurance, consents, and parent contact info. About 10 to 15 minutes.
By phone
Call us and we’ll email or text the forms to you, or we’ll set them aside at the front desk so you can fill them out when you arrive.
At the visit
Prefer to do it in person? Arrive 15 minutes early and our front-office team will help you through everything in English or Spanish.
What to bring
A quick checklist for the day of
Even if you submit forms ahead of time, please bring these in person.
- Photo ID for the responsible adult on the visit
- Insurance card (front and back)
- Immunization records from any prior provider
- List of current medications and any allergies
- Any school, sports, or daycare forms that need a signature
- Birth records if your child is under 1 year old
