Most apply to any Medicaid family. None of them are deal-breakers — call us if anything is missing.

  • Bring your child’s Medicaid card to every visit
  • Bring a photo ID for the responsible adult
  • Tell us your specific managed-care plan when you call (Cardinal Care, Anthem HealthKeepers, Aetna Better Health, Molina, etc.)
  • Request prior records from any other provider — we’ll help with the release form
  • Renew your Medicaid coverage each year — we can remind you when it’s time
