Most apply to any Medicaid family. None of them are deal-breakers — call us if anything is missing.
- Bring your child’s Medicaid card to every visit
- Bring a photo ID for the responsible adult
- Tell us your specific managed-care plan when you call (Cardinal Care, Anthem HealthKeepers, Aetna Better Health, Molina, etc.)
- Request prior records from any other provider — we’ll help with the release form
- Renew your Medicaid coverage each year — we can remind you when it’s time
- Call us with any cost or coverage question before scheduling — we want no surprises
